Another one would be when I was playing a 1 on 1 game with a good friend of mine and I forgot to give her dice. She gets attacked in a random encounter and she's like, "Die..."

Me: "What?"
Her: "DIE, JIMMY!"
Me: "WTF DID I DO?!"
Her: "NO! I need a d10 for initiative!"
